Dance Registration Form

Classes:

• Tiny Tots
  Children ages 3-4 will learn the basic ballet positions with a fun and creative curriculum. Classes use music and movement to introduce dance and coordination.
  
• Ballet & Tap
  Children ages 5-7 and 8-10 will learn classical ballet along with tap. The class combines rhythm, timing, coordination, and style.
  
  
  What to Wear:
  All children are required to wear a long or shortsleeved leotard, tights, and ballet slippers (and tap shoes for Ballet & Tap class).
  
  Class Structure:
  Classes will meet for 45 minutes according to the following schedule:

Registration Fee:  $45  Financial assistance is available.

Print out the following registration form and:
(1) mail it into the YMCA (442 Westfield Road, Charlottesville, VA  22901)
(2) fax it in (974-4651)
(3) drop it by

REGISTRATION INFORMATION:
Participation in all programs requres advance registration.
All programs have enrollment limits.  Space is limited, so register early.
The YMCA accepts Mastercard, VISA, and Discover Card.
YMCA programs are open to all opeople regardless of age, race, religion, gender, ability level, nation of origin, or financial circumstances.  Financial assistance is available.
The YMCA does not send out confirmations.  Once you register for a program, you are in that program unless you hear otherwise.
